Appointments with Dr. Clark are available Monday through Friday 8:00 a.m. to 5:00 p.m. Every effort will be made to schedule your appointment in a timely fashion. All emergencies are addressed immediately and treated accordingly.

Information needed at the Initial Visit:

• Any surgical referral slips and x-rays from your referring dentist or physician
  
• All medical and dental insurance information (cards and/or completed forms)
  
• A list of current medications you are taking
  

Important Notes
All patients under 18 years of age must be accompanied by a parent or legal guardian at a consultation appointment.

Please inform our office if you have any medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, heart murmur, bleeding disorder, etc.) or any drug allergies.


Initial Consult
All patients are required to have an initial consult prior to any procedure. Simple procedures not requiring IV sedation or General Anesthesia can usually be done at this appointment. Complex procedures or those involving IV sedation or General Anesthesia will be discussed in detail and scheduled.


X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our Warren facility.
